RISE is strengths based care coordination. Do you need support making a plan to get resources in place? Together, we then execute the plan and get things done!

High-Fidelity Wraparound is an evidence-based approach to intensive care coordination. It is holistic and values-based for youth with complex needs. We create one unified plan that is driven by the young person's vision, through a structured and facilitated teaming process.

Young people who get involved in multiple systems often struggle. Sometimes it is because the system itself provides confusing, even contradictory messages. This can be a set-up for failure. New Day's care coordination services, both RISE and High-Fidelity Wraparound, restructure the system around a young person and build a functioning team. We seek to create one unified plan that is driven by a young person’s vision, that always includes voice and choice, that understands the needs of system partners, and that is organized around what a young person is willing and realistically able to do. Care Coordination at New Day is a focused way to design success for young people who have primarily known the opposite. The ultimate goal is to help them live successfully in their homes and communities and realize their hopes and dreams.
